
New Patients
If you are a new patient wishing to register with us please see the "practice area" tab for our catchment area for new patients.
In order to register with the practice please complete the necessary 3 forms available below or collect from reception:
You will be asked to provide 1 form of photographic ID and proof of address. Completed applications are currently taking about 7 days to process. Access to practice services can be made after this time.

Existing patients moving out of area
If you are an existing patient who moves out of our practice area you will need to register with another GP Surgery. If you have any questions regarding this please do not hesitate to contact our Office Manager Gillian Jamieson.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with the practice. To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
Children aged 5 to 11 years old are now be eligible for a COVID-19 vaccine. For info click here or call: 0800 030 8013. Please do not call the practice as we are unable to provide information.

Information on what vaccinations are required, together with malarial and safe travel advice can be found at at Home - Fit for Travel. The website allows you to select your destination and then view the requirements for the destination before contacting the providers on the board website. 4 Travel Vaccines are available on the NHS (Typhoid, Hepatitis A, DPT and Cholera) at no direct cost to the patient. Any other vaccinations will incur a cost. Please scan the qr code for more information.
